It is imperative that Trust participants promptly report certain life events to the Trust Administration Office, as these life events may change your benefit coverage. Life events include address changes, adding and removing dependents from coverage, enrolling in Medicare or the death of an enrolled participant. The Trust Administration Office operates 8:00 AM to 5:00 PM EST, Monday through Friday and can be reached toll-free at 1-866-694-6477.
It is very important that participants keep their addresses up-to-date to ensure that they continue to receive Trust communications and other mailings regarding their benefits.
Address changes should be reported to the Trust Administrative Office as soon as possible. Reporting a change of address to the GoodyearBenefitsSolutionCenter will not update your address with the Trust. You may contact the Trust Administrative Office for an Address Update form, or you may click here.
Adding and Removing Dependents from Coverage
Participants are responsible for notifying the Trust when a dependent needs to be added or removed from coverage.
If a dependent becomes eligible for coverage as the result of a qualifying event, the participant must notify the Trust within 30 days of the event and provide any necessary documentation as proof of eligibility. For more information on qualifying events, please visit the Eligibility section under Trust Participant Resources.
If a dependent becomes ineligible for coverage, the participant must remove the dependent from coverage. If a participant fails to remove an ineligible dependent from coverage in a timely manner, the participant may be required to pay for any costs (claims, premiums, etc.) associated with the ineligible dependent. For more information on dependent eligibility, please visit the Eligibility section under Trust Participant Resources.
For more information on adding and removing dependents from coverage, please refer to your Summary Plan Description and/or contact the Trust Administration Office. You may contact the Trust Administrative Office for an Eligibility Change form, or you may click here.
Enrolling in Medicare
Trust participants are required to enroll in Medicare (Parts A and B) when they first become eligible in order to receive the maximum Health Care Plan benefits. When you or your dependents are eligible for Medicare, the Plan pays claims as if you are enrolled in Medicare, whether or not you are actually enrolled. This means that if you are eligible for but not enrolled in Medicare, you will have significantly higher out-of-pocket medical expenses.
Enrollment for Medicare begins three months prior to your 65th birthday and ends three months after that birthday.
Participants must provide a copy of their Medicare card to the Trust Administration Office immediately upon receipt.
Reporting a Death
The death of any participant (retiree, surviving spouse or dependent) must be reported to the Trust.
Notification of death should be reported to the Trust Administrative Office within 30 days of the death. Reporting a death to the GoodyearBenefitsSolutionCenter will not update participant information with the Trust. You will be asked to supply a copy of a certified death certificate to the Trust Administration Office to keep on file.
